The engineer/operative can identify on the form an appliance/installation that is 'Immediately Dangerous' or 'At Risk' by ticking the appropriate box. Up to 4 appliances can be listed on the form. This form is to be used in accordance with the requirements of the current Gas Industry Unsafe Situations Procedure.
The forms come as a set of 3 (NCR) and 25 sets to an A4 pad.
They are colour coded to identify which copy goes to whom.
‘White’ to the gas user/responsible person.
‘Blue’ to the customer or landlord.
‘Pink’ to the registered gas installer.
The forms are sequentially numbered for traceability.
